Helpful information before, between, and after visits.

Find appointment policies, new-patient reminders, portal access, forms, and practical information to help you prepare for care.

Before your appointment.

A little preparation helps your visit run more smoothly. Bring your current insurance card, photo identification, medication and supplement list, relevant records, and any questions you want to remember. If the office asks you to complete forms or send records in advance, please do so by the requested deadline.

New patients

  • Complete and submit new-patient forms at least two business days before your appointment.
  • Plan to arrive 15 minutes early for your first visit.
  • Forms that are not received on time may require the appointment to be rescheduled.

Established patients

  • Please call at least 24 hours before your appointment if you need to reschedule.
  • The current practice policy notes a $65 no-show fee for late cancellations or missed visits.
  • Arriving 10 or more minutes late may require your appointment to be rescheduled.

Forms and online access.

Patient portal

Established patients can use the patient portal for available online account features. For urgent medical concerns, do not rely on a portal message; call the office or follow the practice’s urgent-care instructions.

Patient forms

Download the forms requested for your visit, complete every applicable section, and return them according to the office’s instructions. Do not email sensitive health information unless the practice confirms a secure submission method.

Hospital and childbirth resources.

Patients planning to deliver at StoneSprings Hospital Center may need separate hospital registration and may wish to explore childbirth, newborn-care, or hospital-orientation classes.
